Care Manager
Lisa Graham, MA, LSW has been a social work leader in healthcare for over 25 years, with a special focus on improving transitions of care for individuals and their care partners. Recent experience includes community development and project management for the Metropolitan Area Agency on Aging, where she provided technical assistance to aging services providers on navigating the changing healthcare landscape. Prior to that she spent 17 years with Fairview Health Services directing social work and discharge planning services in hospital and clinic settings, with a special focus on alliance building between healthcare providers and community services. Lisa brings expert knowledge of community services to assist older adults in maintaining their independence in the community, and is passionate about helping people achieve their goals. She holds a Bachelor of Science in Social Work from St. Catherine University, and Master of Arts in Human Development from St. Mary’s University, with an emphasis in end of life planning. She has a Certificate in Aging Studies from Boston University School of Social Work and is an active member of the Minnesota Gerontological Society.
